Successfully completing a short-term rehabilitation program at a facility like Troy Center for Rehabilitation and Nursing is an exciting time, but we also want to make sure that it’s a smooth transition.

Being prepared for returning home is the key to making sure that you continue to get better once you leave a situation where you are being monitored 24 hours a day, seven days a week. Whether you’re going back to living alone or having a family member help with your care, following these four tips will give you the confidence you need to know that you will have a smooth transition back to your home.

  1. Consider Having a Home Safety Assessment Done

You’ll want to make sure your home is safe to return to. You may need modifications like grab bars, non-slip surfaces in tubs, and extra stair railings installed. Older homes may also have steep or uneven staircases and poor lighting. Ask a physical or occupational therapist to conduct a home safety assessment so possible hazards are addressed and corrected before you return to the home.

2. Follow Doctor’s Orders

You will receive follow-up orders from a doctor upon your discharge from the rehab facility. These orders may include further out-patient therapy and follow-up visits to physicians. Don’t skip any of these orders because your continued recovery is dependent upon what the doctor feels will be best for you.

3. Continue to Work with Your Rehab Team

The rehab team will likely give you goals to work toward once you return home — these can range from fitness milestones to meal planning. The team will help you set reasonable goals so you do not become discouraged.

4. Manage Medications

Medication management is one of the many things that is done for you in a short-term rehabilitation facility. There are simple things you can do like setting alarms on your phone or having a family member text or call you as reminders to take your medicine. Be sure the staff at the rehab center lists all your medication and how often you need to take it.

On top of all these tips, be sure to stay in touch with your rehab facility. Call whenever you have a question or concern.
